Program Admission Consideration

Adult & Teen Challenge offers a custom program that offers mental health, physical, spiritual and substance abuse services. Due to the client commitment needed to participate in a program of this length and our commitment to high quality of care please provide all of the following so that we can determine appropriate placement.

Medical Records that include:

• HPI with diagnosis
• All labs
• List of current medications
• Current provider/nursing notes/case notes
• Additional Tests and Studies: To include but not limited to X-rays, CT, EKG, MRI
